PENGARUH NILAI ‘B’ VALUE TERHADAP CITRA MRI BRAIN KASUS STROKE ISKEMIK HIPERAKUT SEQUENCE DWI (DIFFUSION WEIGHTED IMAGING)

Abstract

Tuberkulosis (TB) menimbulkan persoalan serius bagi Indonesia, serupa dengan Salah satu pulse sequence MRI Brain adalah Diffusion Weighted Imaging (DWI). Sequence ini merupakan gold standard pada modalitas pencitraan MRI untuk diagnosis stroke iskemik dengan sensitivitasnya yang tinggi. Parameter terpenting pada sequence ini adalah nilai 'b' value. Nilai default untuk ‘b’ value adalah 1000 s/mm2 namun, masih belum jelas apakah nilai 'b' 1000 s/mm' lebih baik dari nilai 'b' lebih tinggi untuk mendeteksi lesi iskemik dan luasnya, terutama pada 6 jam pertama setelah timbulnya stroke iskemik. Seiring dengan peningkatan nilai ‘b’ value maka akan adanya peningkatan sinyal untuk menaikkan sensitifitas sinyal difusi, namun hal tersebut juga mengkibatkan bertambahnya noise. Sebanyak delapan pasien diberikan tiga perlakuan nilai ‘b’ yang berbeda, masing-masing slice. Kemudian dilakukan penilaian terhada tiga dokter spesialis radiologi, selanjutnya uji statistik dilakukan untuk memperoleh nilai optimal dalam pemilihan ‘b’ value. Hasil penelitian menunjukkan terdapat perbedaan yang signifikan antara ketiga variasi nilai ‘b’ yang berbeda. Kualitas citra dan anatomi informasi terbaik pada pemilihan ini adalah “b’ value 2000 s/mm2.
