HUBUNGAN ANTARA KELENGKAPAN DENGAN KETEPATAN WAKTU PENGISIAN FORMULIR ASESMEN AWAL PASIEN RAWAT INAP PENYAKIT MALIGNANT NEOPLASM OF CERVIX UTERI, UNSPECIFIED DI RSUD DR. MOEWARDI TRIWULAN I TAHUN 2018

Abstract

Kelengkapan dan ketepatan waktu pengisian rekam medis merupakan konsep penjaminan mutu rumah sakit di RSUD Dr. Moewardi. Asesmen awal sebagai penyedia informasi utama penting untuk segera dilengkapi di rumah sakit. Berdasarkan survey pendahuluan terhadap penyakit kanker serviks, prosentase paling tidak lengkap adalah 58,97% pada review pencatatan, sedangkan prosentase tidak tepat waktu mencapai 30%. Penelitian ini bertujuan untuk mengetahui kelengkapan dengan ketepatan waktu pengisian formulir asesmen awal pasien rawat inap penyakit malignant neoplasm of cervix uteri, unspecified di RSUD Dr. Moewardi Surakarta triwulan I tahun 2018. Penelitian ini analitik menggunakan metode kuantitaif dengan pendekatan cross sectional study. Sampel penelitian diambil dengan tekhnik sampel jenuh yaitu seluruh Formulir Asesmen Awal Pasien Rawat Inap Penyakit Malignant Neoplasm of Cervix Uteri, Unspecified di RSUD Dr. Moewardi Surakarta Triwulan I Tahun 2018 sebanyak 195 formulir. Instrumen penelitian menggunakan pedoman observasi, pedoman wawancara, dan checklist.. Analisis data kuantitatif menggunakan uji hubungan chi-square.  Hasil penelitian menunjukkan bahwa sebagian besar formulir ditemukan tidak lengkap sebanyak 115 (58,97%) dan masih ditemukan ketidaktepatan waktu pengisian 1x24 jam pada formulir sebanyak 61 formulir (31,28%). Hasil penelitian ini dari 195 formulir berdasarkan hasil uji Chi-Square signifikansi p anatara variabel kelengkapan dan ketepatan waktu pengisian formulir asesmen awal diketahui nilai ? sebesar sebesar 0,003 < ? (0,05),  maka Ho ditolak dan dinyatakan ada hubungan antara kelengkapan dengan ketepatan waktu pengisian formulir asesmen awal pasien rawat inap penyakit malignant neoplasm of cervix uteri, unspecified di RSUD Dr. Moewardi Surakarta triwulan I tahun 2018. Kesimpulan yang diperoleh penelitian ini adalah ada hubungan antara kelengkapan dengan ketepatan waktu pengisian formulir asesmen awal pasien rawat inap penyakit malignant neoplasm of cervix uteri, unspecified di RSUD Dr. Moewardi  Triwulan I Tahun 2018.

 The completeness and accuracy of the timing of filling out the medical record is the concept of hospital quality assurance in RSUD Dr. Moewardi. The initial assessment as an important provider of key information is to be completed at the hospital immediately. Based on a preliminary survey of cervical cancer, the most incomplete percentage was 58.97% in the recording review, while the percentage was not on time reaching 30%. This study aims to determine the completeness with the timeliness of filling in the initial assessment form of inpatients with malignant neoplasm of uterine cervical disease, unspecified in Dr. Hospital. Moewardi Surakarta 1st quarter 2018. This research is analytical using a quantitative method with a cross sectional study approach. The research sample was taken with a saturated sample technique, namely the entire Initial Assessment Form of Inpatient Patients with Malignant Disease Neoplasm of Cervical Cervix, Unspecified in Dr. Hospital. Moewardi Surakarta 1958 First Quarter of 195 forms. The research instrument used observation guidelines, interview guidelines, and checklist. Quantitative data analysis used the chi-square relationship test. The results showed that most of the forms were found to be incomplete as many as 115 (58.97%) and still found an inaccurate filling time of 1x24 hours on the form as many as 61 forms (31.28%). The results of this study of 195 forms based on the Chi-Square test the significance of the completeness and timeliness of filling out the initial assessment form are known to be ? of 0.003 <? (0.05), so Ho is rejected and there is a relationship between completeness and timeliness filling in the initial assessment form for inpatients of malignant neoplasm of uterine cervix, unspecified at Dr. Moewardi Surakarta in the first quarter of 2018. The conclusion of this study is that there is a relationship between completeness and the timeliness of filling out the initial assessment form for inpatients in malignant neoplasm of uterine cervical disease, unspecified in Dr. RSUD. Moewardi Quarter I Year 2018.
